Your Eyelids, Your Shape

Every beautiful Asian eyelid is individual, but each one is shaped by three key features: the shape of the upper eyelid crease, the height of the pretarsal show (the visible lid between the lashes and the crease), and the upper lid fold that sits above the crease.

“Double eyelid surgery” is the term most often used for younger Asian patients who want to create a crease where there is none (a monolid) or to reshape an existing one. “Asian blepharoplasty” is often used for more mature patients seeking eyelid rejuvenation, which usually involves removing excess skin as well as defining the crease.

Additional procedures can refine the result further. Epicanthoplasty adjusts the epicanthal fold at the inner corner of the eye, while a sub-brow lift can reduce fullness and heaviness in the upper lid.

The Three Types of Asian Eyelid Surgery

There are three different types of Asian eyelid surgery:

  • First: Double eyelid surgery for younger Asians who wishes to create or change the shape of the double eyelid. Read more
  • Second: Asian blepharoplasty with or without brow lift for mature Asians who wants eyelid rejuvenation.
  • Third: Thread lift (non-surgical) for selected Asians both young and mature.

Sub-Brow Lift & Asian Blepharoplasty

When sagging upper eyelid skin is caused by the heavy eyebrows, the best solution is to lift the upper eyelid skin from the eyebrow.

This is called sub-brow lifting and Dr Kim has published his technique in the American Journal of Cosmetic Surgery titled "Combined Sub-Brow Lift and Upper Blepharoplasty to Minimize the Supratarsal Fold Thickness and Lateral Hooding: A Concept and Review of Cases". In those with heavy eyebrows, it can produce a more naturally appearing result.

What does the surgery involve?

Double eyelid surgeries are a popular surgical procedure for Asians who wish to create or reshape their double eyelids. It is Dr Kim's intention to create the double eyelids non-surgically whenever possible. A surgery is required where there is excess skin hooding and where fat removal is desirable.

It is a procedure in which the upper eyelid crease is created or reshaped by removal of excess skin, removal of fat, creating double eyelids and stitching the skin. In some patients, where there is droopy eyebrows, sub-brow lifting will be recommended at the same time.

The procedure is performed under local anaesthesia and usually takes about 60 minutes. It can be done in our clinic. The result is permanent.

Complications

There are a number of complications that are generally associated with all surgeries, and some specific to upper blepharoplasty. Most complications are temporary and heal spontaneously. Some of the things that could go wrong are:

Scarring — unavoidable; we place the scar in the crease to make it less perceptible.
Pain — usually paracetamol is sufficient.
Prolonged swelling — lasting for months and in rare event up to a year.
Bleeding after discharge — need to return to the clinic to stop the bleed.
Asymmetry — 10% normal, requiring correction.
Abnormal scars — requiring scar injections and other corrections.
Blurred vision — usually few days due to swelling.
Dry eyes — need some lubricating eyedrops.
Over and under correction — careful planning to avoid this.
Not meeting patient's expectations
Infection — we routinely give antibiotics.
Allergic reaction
Double eyelids gets undone
Blindness and death — we are not aware of any cases from blepharoplasties performed under local anaesthesia; these complications are usually related to general anaesthesia, under which they were reported.

A qualitative retrospective analysis of before and after photos of 268 cases of combined sub-brow lift and upper blepharoplasty. A substantial number of cases had satisfying supratarsal fold thickness and improvement in lateral hooding — an effective method for Asians with moderate to severe blepharochalasis.

Durable Suture Technique: A Simple and Versatile Nonsurgical Technique to Create/Reshape Double Eyelid in Asians. The American Journal of Cosmetic Surgery, Jan 2023.

The Durable Suture Technique is a popular nonsurgical procedure for creating double eyelids on monolids — effective in reshaping and rejuvenating double eyelids, as well as treating post-blepharoplasty complications.

Mandatory two consultations are required before the surgery, along with a cooling-off period, a Body Dysmorphic Disorder Assessment, and a referral from your GP.
